		The following forms are all required for participation in the Marlborough Athletic Program. Completed and signed forms must be filed with the Athletic Director before athletes can participate in tryouts/practices or competition. Physical forms are valid for 1 school year from June 1st to June 1st. (Physicals taken before June 1st, 2001 are not valid for the 2001/2002 school year.) Please complete both pages and obtain the required doctor's signature before mailing them c/o the Athletic Department. (Please do not fax documents as they are often unreadable.) Preseason physicals are mandatory by CIF.
